We have changed to a new electronic medical records software and a new account will be created for all our patients to be able to access all the features explained below.   

We are rolling out everyone's new user name in small groups to avoid an overwhelming stress on our resources to assist patients with log-in questions.   

If you would like access to your health information and all the features ​before your next appointment, please call the office.

As a patient you receive access to our Patient Portal that will allow you to securely:
  • Request an appointment
  • See future and past appointments
  • Request Rx refills
  • See past medical notes
  • Request medical records
  • Ask the nurse a question 
  • Download forms
  • Update demographics
